Clinical Guide, RN

Responsibilities

• Perform clinical triage to assess patient health concerns using evidence-based protocols
• Provide clinical navigation support including condition education, care planning, and medication adherence
• Document symptoms, nursing assessments, and guidance provided accurately
• Conduct proactive outreach to high-risk and rising-risk members to assist with lifestyle changes
• Coordinate screening services and provide care management follow-up
• Act as a health advocate to direct members to high-quality providers and appropriate care settings

Requirements

• RN, ADN, or BSN degree
• Active RN license in a compact state and residency in a compact state
• 5+ years of clinical experience

Preferred Qualifications

• Experience in family medicine
• Triage nursing experience
• Care management or chronic condition support experience
• Multiple RN state licenses (CA, NY, MI, MA, MN)
