Clinical Pharmacist

Responsibilities

• Provide integrated support to patients and physicians to improve medication adherence and promote evidence-based care
• Review clinician assessments for prior authorization approvals and care plan recommendations
• Develop and refine disease and drug-specific clinical protocols
• Monitor patient adherence using PDC reports and mobile platform data, focusing on oncology and specialty disease states
• Evaluate EMR data including drug interactions, medication selection, labs, and vitals
• Perform Medication Therapy Management (MTM) and recommend therapeutic drug regimen changes
• Conduct clinical interventions including motivational interviewing, medication reconciliation, and drug regimen optimization
• Oversee technician activities including delivery coordination and prior authorization logs
• Identify and resolve insurance-related issues and financial assistance opportunities
• Maintain compliance with HIPAA and state/federal pharmacy laws

Requirements

• Doctor of Pharmacy (PharmD) or equivalent advanced degree
• Licensed Pharmacist in South Carolina AND (Georgia OR Tennessee)
• Proficiency in Microsoft Office (Word, Excel, PowerPoint, Outlook, Teams)
• Strong technical and clinical skills with ability to manage complex processes
• Ability to identify barriers to medication adherence and coach patients

Preferred Qualifications

• 1+ years of pharmacy experience
• Knowledge of EPIC, QS1, or WILLOW software
• Expertise in specialty pharmacy areas such as Oncology, Transplant, Hepatitis C, HIV, Multiple Sclerosis, or Rheumatoid Arthritis
