Community Health Worker Certified-3

Responsibilities

• Provide patient-centered outreach to individuals with identified social needs
• Collaborate with healthcare teams and community-based organizations to coordinate support
• Utilize motivational interviewing and problem-solving to connect patients to resources like food, housing, and transportation
• Facilitate referrals through the Neighborly platform
• Conduct follow-up outreach to confirm successful connections
• Document all efforts and outcomes in the electronic medical record (EMR)
• Assist in identifying and outreaching medically complex patients
• Perform disease-specific education and medication reviews
• Conduct safety evaluations in patients' homes
• Facilitate telemedicine visits

Requirements

• High School Diploma or Equivalent (GED)
• Minimum of 1 year of related work experience
• Certified Community Health Worker (CCHW) - Pennsylvania Certification Board
