Community Health Worker

Responsibilities

• Conduct telephonic and virtual outreach to patients with high SDOH needs
• Serve as a liaison between patients, families, clinical teams, and community providers
• Assess social determinants of health and connect patients to resources
• Coordinate care transitions and assist with enrollment in health and social services
• Track referrals and outcomes for pharmacy, DME, and home care services
• Maintain compliant documentation of patient interactions and care plans
• Provide education to empower patients and families in managing health conditions

Requirements

• 3+ years of experience in healthcare coordination or community health work
• Active Community Health Worker certification
• Experience with team-based care and strong knowledge of community resources
• Intermediate skills with MS Office Suite including Outlook and Teams
• Reliable home internet (25 Mbps download / 10 Mbps upload)
• Dedicated remote workspace that ensures HIPAA compliance

Preferred Qualifications

• Bilingual proficiency in Spanish and English
