Community Health Worker

Responsibilities

• Provide patient-centered outreach primarily through telephonic engagement to individuals with identified social needs
• Collaborate with healthcare teams and community-based organizations to assess needs and coordinate support
• Utilize motivational interviewing to connect patients to resources for food, housing, transportation, and utilities
• Facilitate referrals through the Neighborly platform and conduct follow-up outreach
• Document outreach efforts and outcomes within the electronic medical record (EMR)
• Perform disease-specific education and medication reviews to ensure adherence
• Conduct safety evaluations in patients' homes and facilitate telemedicine visits

Requirements

• High School Diploma or Equivalent (GED)
• Minimum of 1 year of related work experience
• Certified Community Health Worker (CCHW) certification from the Pennsylvania Certification Board
• Proficiency in communication, multitasking, and problem solving
