Crisis Care Manager

Responsibilities

• Provide emotional support and encouragement to patients and families experiencing behavioral health crises
• Handle case referrals, assess resourcing needs, and determine case closure
• Assist families in recovery goals through psychoeducation and community resource linkage
• Assess social determinants of health including housing, food insecurity, and transportation
• Connect families with support groups and mental wellness networks
• Collaborate with interdisciplinary teams to facilitate treatment plan goals
• Coordinate care with agencies such as DCFS and school systems
• Participate in scheduled consultations to monitor patient progress

Requirements

• Master’s degree in Social Work (MSW)
• 2+ years of case management or relevant experience
• Experience with acute behavioral health needs (suicidality, homicidality, severe persistent mental illness)
• Experience in child & adolescent case management, school-based care, or inpatient settings
• Strong knowledge of community resources
• Excellent collaboration skills for team-based care

Preferred Qualifications

• Bilingual in English/Spanish

• Base salary range of $70,000 - $77,000 plus annual bonus
