Health Coach

Responsibilities

• Provide remote patient care on Revero’s app via texting, phone calls, and video chats
• Provide education and coaching to assist patients in implementing individualized nutrition and care plans
• Address patient progress with medical staff and communicate physician recommendations to patients
• Monitor and provide feedback on daily metrics including symptoms, energy, cravings, hunger, and mood
• Empathize and partner with patients to ensure personal success and sustainability
• Review and update medications, medical history, and laboratory values while documenting interactions in the portal

Requirements

• Four-year college degree in Nutrition, Dietetics, Kinesiology, Exercise Science, Nursing, Social Work, Psychology, or a related health field
• Professional degree or certification such as Registered Dietitian, Registered Nurse, or Registered Exercise Physiologist
• Outstanding verbal and written communication skills
• High EQ and empathy with a strong desire to build patient relationships
• Strong technological literacy for digital and virtual care platforms
• At least 1 year of prior coaching experience

Preferred Qualifications

• Experience in outpatient nutrition counseling or motivational interviewing
• Experience implementing behavior change techniques or mental health counseling
• Ability to commit 20-28 hours per week with flexible hours
• Comfort working independently in ambiguous environments
