Home Health Registered Nurse - Triage

Responsibilities

• Triages all patient and facility calls after hours
• Facilitates new and existing home health referrals
• Acts as a liaison for community education
• Collaborates with physicians to obtain orders
• Provides guidance and support to field staff
• Assists in scheduling patient admissions and visits
• Monitors telehealth as needed

Requirements

• Associate or Bachelor's degree in Nursing
• Two years of clinical or home health experience preferred
• Active and unencumbered Multi-State Registered Nurse license
• Alaska, California, and Michigan RN licensure required
• Current CPR certification
• Strong mathematical and communication skills
