Licensed Practical Nurse

Responsibilities

• Conduct outbound telephonic patient outreach and provide one-on-one coaching for chronic conditions like type 2 diabetes and hypertension
• Help patients execute personalized care plans through education on nutrition, physical activity, and self-management
• Conduct comprehensive assessments of health status, lifestyle behaviors, and nutritional habits
• Educate patients on disease management, medication adherence, and symptom recognition
• Monitor patient progress and adherence to treatment plans using remote monitoring tools
• Document all patient interactions and identify barriers to care in the Cadence platform
• Collaborate with clinicians and support teams using Google Workspace and Slack

Requirements

• Active compact multi-state LPN/LVN license in current state of residence
• Active CA state LVN license
• At least 5 years of clinical experience as a Licensed Practical Nurse
• Remote work readiness with stable high-speed WiFi

Preferred Qualifications

• Fluency in Spanish
• Experience providing patient education for chronic condition management
• Fluency with AI-assisted clinical tools such as Gemini or Open Evidence
