LPN - Nurse Navigator Licensed Practical Nurse - Population Health Nurse Triage

Responsibilities

• Perform duties as a patient navigator guiding incoming calls and facilitating entrance of new patients into the program
• Provide patient education and promote understanding of conditions
• Coordinate patient needs and activities such as support groups and educational series
• Collaborate with healthcare providers to implement plans of care
• Maintain clinical supply inventory and conduct medical record reviews
• Assist physicians with procedures and provide nursing care

Requirements

• Graduate from Specialty Training Program-Nursing
• Minimum of 2 years previous LPN work experience
• Licensed Practical Nurse certification/license
• Proficient computer skills including Microsoft Word, Excel, PowerPoint, and Teams
