Cadence is the clinical AI company automating the treatment of chronic disease. Its Clinical Intelligence takes on the routine, high-volume work of chronic care – monitoring patients, surfacing risks, and coordinating action – so clinicians can focus on the decisions that require them.

Responsibilities

• Conduct outbound telephonic patient outreach and provide one-on-one coaching for chronic conditions like type 2 diabetes and hypertension
• Help patients execute personalized care plans through education on nutrition, physical activity, and behavior modification
• Conduct comprehensive assessments of health status, lifestyle, and nutritional habits
• Educate patients on disease management, medication adherence, and symptom recognition
• Monitor patient progress and adherence to treatment plans using remote monitoring tools
• Document patient interactions and identify barriers to care in the Cadence platform
• Collaborate with clinicians and support teams using Google Workspace and Slack

Requirements

• Active Medical Assistant certification from an accredited association (AAMA, AMCA, AMT, NHA, NAHP, NCCT, NRCMA/NRCAMA, or AAH)
• At least 5 years of experience as a Medical Assistant
• Remote work readiness with stable high-speed WiFi
• Fluency with AI-assisted clinical tools

Preferred Qualifications

• Experience in internal medicine, cardiology, nephrology, or with geriatric populations
• Experience providing patient education for chronic condition management
• Fluency in Spanish
