Job description

Responsibilities

  • Conduct outbound telephonic patient outreach and provide one-on-one coaching for chronic conditions
  • Educate patients on disease management, medication adherence, and prevention strategies
  • Document patient interactions and monitor progress through remote monitoring tools

Requirements

  • Active Medical Assistant certification from an accredited association
  • At least 5 years of experience as a Medical Assistant
  • Fluency in Spanish

Preferred Qualifications

  • Experience in internal medicine, cardiology, or nephrology
  • Experience working with geriatric populations
  • Proficiency with AI-assisted clinical tools
