Responsibilities

• Manage a caseload of low- to moderate-acuity clients, providing education, advocacy, care coordination, and ongoing support
• Gather and synthesize relevant clinical information and prepare clear summaries for internal teams
• Collaborate cross-functionally to coordinate client handoffs and maintain continuity
• Research and recommend appropriate providers and facilities based on client needs
• Coordinate appointments, records, referrals, and other logistics to support seamless care delivery
• Communicate clearly and compassionately with clients, caregivers, providers, and internal partners

Requirements

• Active Registered Nurse (RN) license in your state of residence
• At least five years of healthcare experience
• Experience in case management, care coordination, patient navigation, or a related clinical environment
• Ability to work Monday through Friday, 8:30 a.m. to 5:00 p.m. Pacific or Mountain Time
• Experience working remotely
• Strong written and verbal communication skills
