Nurse Care Manager - Compassionate Care Program

Responsibilities

• Build empathetic relationships with patients and families
• Complete patient education regarding diagnosis, prognosis, and treatment options
• Conduct advance care planning (ACP) and Goals of Care (GOC) discussions
• Identify opportunities for symptom palliation and develop care plans
• Coordinate care and access to health services across multiple providers including home health, palliative care, and hospice
• Act as a clinical liaison and educator for internal and external partners
• Maintain accurate patient records and implement care plans across settings

Requirements

• Active RN license in good standing
• 3+ years of clinical practice in an intensive care unit, hospice, or palliative care role
• Experience conducting goals of care and end-of-life care discussions
• Ability to work effectively in a remote environment with required internet speeds
• Intermediate skills with MS Office Suite

Preferred Qualifications

• CHPN (Certified Hospice and Palliative Care Nurse)
• Compact RN licensure
• BSN
• 1+ years of utilization management experience
