Nurse Practitioner or Physician Assistant

Responsibilities

• Perform preventative care screenings, medication reconciliation, and diagnose medical conditions
• Provide health coaching, referrals, and treatment plans to insurance members
• Conduct virtual outreach to chronically ill members and those with low PCP engagement
• Identify opportunities to improve member access to care through licensed providers
• Cross-train into urgent care and virtual primary care service lines as needed
• Ensure compliance with all applicable healthcare laws and regulations

Requirements

• PA or DNP, FNP, ENP degree from an accredited program
• Board Certification (AANP, ANCC, or NCCPA)
• 3+ years of outpatient experience in Family Medicine, Internal Medicine, or Primary Care
• Spanish fluency (ability to read, write, and speak)
• For NPs: Compact RN and licensure in at least 2 of the following: FL, TX, OH, NJ
• For PAs: Licensure in GA and one of the following: FL, TX, OH
• Experience with Clinical Documentation Improvement and/or HEDIS quality measures
• Ability to obtain licensure in additional states

• Base pay: $130,000 - $140,000 per year plus annual performance bonuses
