Job description

Responsibilities

  • Perform preventative care screenings, complete medication reconciliation, and diagnose medical conditions.
  • Provide health coaching, referrals, and treatment to members virtually.
  • Conduct outreach to chronically ill members and those with a lower probability of seeing their PCP.
  • Identify opportunities to improve member access to care through licensed providers.
  • Cross-train to support Virtual Urgent Care and virtual primary care service lines.
  • Ensure compliance with all applicable laws and regulations.

Requirements

  • PA or DNP, FNP, ENP from an accredited program.
  • Board Certification (AANP, ANCC, or NCCPA).
  • 3+ years of experience in outpatient Family Medicine, Internal Medicine, or Primary Care.
  • Experience with Clinical Documentation Improvement and/or managing HEDIS quality measures.
  • For NPs: Compact RN and licensure in Florida plus either Texas or Ohio.
  • For PAs: Licensure in Georgia and one of the following: Florida, Texas, or Ohio.
  • Ability to obtain licensure in additional states.
