Nurse Practitioner

Responsibilities

• Provide call-based care to members in the Remote Patient Monitoring (RPM) program
• Conduct phone visits for initial member consultations and evaluate diagnoses, medications, and lab results
• Identify and prescribe Continuous Glucose Monitors (CGMs) for applicable members
• Serve as the primary clinical point of contact for practice-wide escalations
• Collaborate with RNs, RDs, and Accountability Coaches to reinforce clinical goals
• Adhere to company policies and standard operating procedures to ensure clinical quality

Requirements

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
• Active certification as a Nurse Practitioner
• Valid, unencumbered multi-state nurse practitioner licensure (priority in AL, UT, IA, IL, IN, MI, GA, FL)
• Minimum 3+ years of experience managing patients with diabetes
• Ability to work remotely in the United States with a quiet workspace and solid internet
• Proficiency with technical tools including computers, software, and phone systems

Preferred Qualifications

• Familiarity with continuous glucose monitors (CGMs)
• Knowledge of behavior change theory
• Training in Motivational Interviewing
• Fluency in Spanish
