Responsibilities

• Conduct thorough assessments of cancer survivors via telemedicine to identify physical, emotional, and psychosocial needs
• Provide evidence-based guidance and education, collaborating closely with the survivorship care team
• Develop and update survivorship care plans in collaboration with patients and behavioral health professionals
• Coordinate and communicate effectively with internal and external care teams, including oncologists and PCPs
• Provide education and support to patients regarding cancer survivorship and ongoing care needs
• Work with clinical leadership to build and enhance survivorship care programs

Requirements

• 3-5+ years of advanced practice experience in an oncology healthcare setting
• Master’s degree in a relevant course
• Current certification as a Nurse Practitioner or Physician Assistant
• Ability to obtain licensure as an APP in specified states
• Proficiency in electronic medical records documentation
• Full-time availability for a 40-hour workweek, Monday to Friday

Preferred Qualifications

• Experience in providing cancer survivorship care
• Strong symptom or supportive care management experience
