Partner Success Manager

We partner with health plans to make homes safer and more accessible for older adults and high-needs populations by combining technology with networks of clinicians, suppliers, and installers.

Responsibilities

• Serve as the relationship owner for active partnerships, maintaining regular communication and ensuring partner satisfaction
• Act as the "voice of the partner" internally, translating feedback into actionable insights for product and operations teams
• Drive growth and expansion within existing partnerships by identifying opportunities for program enhancement
• Own visibility into key metrics like referral volume, SLA trends, and utilization to drive strategic interventions
• Lead end-to-end implementation planning and execution for new partnerships from contract to launch
• Coordinate cross-functional teams including operations, clinical, and technical resources

Requirements

• Clinical degree or licensure (OT or related healthcare credential)
• 3+ years of project management, consulting, or partner management experience in healthcare
• Experience with managed care organizations, Medicaid, or healthcare payers
• Demonstrated experience managing complex implementations or program launches
• Strong analytical skills to interpret performance and program data
• Excellent communication skills to translate clinical and operational concepts

Preferred Qualifications

• Prior experience with NY Medicaid HCBS programs
• Knowledge of home and community-based services (HCBS) and population health
• Experience with healthcare technology implementations
• Experience working in a fast-paced startup environment
• Resides in the NY tristate area
