Patient Care Coordinator

Responsibilities

• Act as the primary point of contact between patients and the care team
• Triage and resolve clinical messages from patients to clinicians
• Conduct regular outgoing patient communications to ensure engagement
• Collect, request, and review patient medical records
• Facilitate medical records, prescriptions, and lab requisitions
• Assist with prior authorization coordination and medication refills
• Guide new patients through the weight loss program and address general questions
• Support patients with account changes, cancellations, and scheduling
• Assist with insurance, billing, and membership eligibility questions

Requirements

• Medical Assistant (MA) Certificate (CMA, RMA, or CCMA)
• At least two years of experience in primary care, obesity medicine, or endocrinology
• Excellent customer service and communication skills
• Highly organized with ability to manage multiple tasks
• Proficient in Slack, Google Workspace, and adapting to new IT systems

Preferred Qualifications

• Experience with prescription prior authorization processes
