Patient Engagement Specialist

Responsibilities

• Own proactive outbound outreach to schedule patient appointments with clinicians and care team members
• Drive patient adherence to daily vital sign monitoring through personalized engagement
• Identify and re-engage patients at risk of disengaging from the program
• Resolve patient questions, concerns, and objections regarding appointments and monitoring
• Coordinate with clinical and operational teams for timely follow-up on at-risk patients
• Document all patient interactions and engagement outcomes in internal systems
• Deliver compassionate support to strengthen the patient experience

Requirements

• 2+ years of experience in sales, customer service, patient outreach, or healthcare support
• Proven track record of delivering service to patients or customers
• Excellent verbal and written communication skills
• Ability to meet performance metrics and engagement goals
• Proficiency with CRM platforms and Mac-based software
• Fluency with AI productivity tools like Gemini or Notion AI
