Job description

Responsibilities

  • Perform preventative care screenings, complete medication reconciliation, and diagnose medical conditions
  • Provide health coaching, referrals, and treatment when clinically appropriate via virtual assessments
  • Conduct outreach to chronically ill members and those with a lower probability of seeing their PCP
  • Identify opportunities to improve member access to care within their medical journey
  • Cross-train into urgent care and/or virtual primary care service lines as needed
  • Ensure compliance with all applicable laws and regulations

Requirements

  • Physician Assistant degree from an accredited program
  • NCCPA Board Certification
  • 3+ years of outpatient experience in Family Medicine, Internal Medicine, or Primary Care
  • Bilingual fluency in Spanish (reading, writing, and speaking)
  • Active licensure in Georgia AND one of the following: Florida, Texas, or Ohio
  • Experience with Clinical Documentation Improvement and/or managing HEDIS quality measures
  • Ability to obtain licensure in additional states
