Form Health is a virtual obesity medicine clinic delivering multi-disciplinary evidence-based obesity treatment through telemedicine. We leverage technology to enhance the patient experience, providing high-quality expert care through a physician-led team including advanced practice professionals and Registered Dietitians.

Responsibilities

• Provide direct patient care focusing on evidence-based obesity treatment and chronic weight management
• Collaborate with APP colleagues to co-manage shared patient panels
• Review charts and conduct return visits for APP patients every third visit to ensure quality
• Provide treatment planning and medical management support for complex cases
• Prescribe non-controlled obesity medications
• Conduct weekly collaborative meetings and asynchronous communication with assigned APPs
• Assist in the professional development of APPs through mentorship and feedback
• Participate in team-based care initiatives and quality improvement efforts

Requirements

• MD or DO degree
• Board certified and diplomate of the American Board of Obesity Medicine (ABOM)
• Minimum of 5 years as an attending physician in primary specialty
• Active and unrestricted state license in TX or WI (or specific IMLC requirements for AL, KS, IA, MN, LA, NE)
• Current or previous credentialing with insurance payers
• Experience working with APPs, medical residents, or trainees in a clinical academic setting
• Commitment to evidence-based obesity treatments, nutrition, and pharmacotherapy
