Form Health is a virtual obesity medicine clinic delivering multi-disciplinary evidence-based obesity treatment through telemedicine. Founded in 2019, Form Health is a venture-backed innovative startup dedicated to empowering patients and leading the field of Obesity Medicine.

Responsibilities

• Provide direct patient care in a clinical setting, focusing on evidence-based obesity treatment and chronic weight management
• Provide treatment planning and medical management support for complex cases
• Prescribe non-controlled obesity medications for patients under direct care
• Ensure clinical documentation, protocols, and treatment pathways are followed consistently
• Assist in the professional development of APPs by offering feedback and supporting continuing education
• Participate in a rotating after-hours call schedule

Requirements

• MD or DO degree with board certification
• American Board of Obesity Medicine (ABOM) diplomate or board certified in Endocrinology
• Licensed to practice and prescribe in AL, TN, OK, MO, IN, SC, or GA
• Must currently hold a Letter of Qualification (LOQ) with the IMLC
• Experience conducting telemedicine visits
• Experience working collaboratively with APPs and registered dietitians

Preferred Qualifications

• Credentialed with multiple insurance payers
• Bilingual with medical fluency in Spanish
