Primary Care Physician

Responsibilities

• Provide preventative, acute, and chronic primary care via fully remote video visits
• Manage a dedicated patient panel with a focus on longitudinal relationships
• Refer patients to specialists or acute care services when clinically indicated
• Execute population health initiatives including AWVs, care gaps, and transitions of care
• Support post-discharge patients to improve treatment plan adherence and reduce readmissions
• Practice continuous self-evaluation to ensure adherence to clinical guidelines and best practices

Requirements

• Board-certified in Family Medicine or Internal Medicine
• Active Wisconsin, Minnesota, and Iowa medical licenses with IMLC compact licensure
• 3+ years of post-residency work experience
• 2+ years of experience in an in-person primary care setting
• Prior experience with virtual visits and EPIC EMR
• Clean background and medical malpractice history
• Ability to commit 40 hours per week, including Monday and Friday

Preferred Qualifications

• Spanish language proficiency
• Experience handling 3+ cases per hour in a virtual setting
• Strong interest in chronic care management and longitudinal patient relationships
• Comfort managing acute conditions via telemedicine
