Program Manager, Chronic and Complex Care

Responsibilities

• Oversee day-to-day operations for the CCPM care management teams
• Ensure delivery of high-quality longitudinal chronic/complex care management
• Evaluate and improve the performance of the CCPM program and team members
• Develop and implement collaborative strategies to improve patient outcomes
• Support newly hired CCPM team members through orientation and onboarding
• Perform regular call and documentation audits
• Prepare performance reports and dashboards
• Develop and foster relationships with key stakeholders
• Build and maintain effective communication strategies
• Cultivate a culture of high performance, transparency, and trust

Requirements

• Bachelor's degree in healthcare or nursing (BSN required)
• Active state nursing license in good standing
• 3+ years of longitudinal clinical nursing care management experience
• 2+ years of experience in a clinical nursing managerial or leadership role
• Experience in longitudinal care management
• Strong business acumen of value-based healthcare operations
• Understanding of Business Continuity and resiliency
• Ability to develop talent and delegate effectively
• Exceptional relationship-building and communication skills
• Strong leadership presence
• Execution-focused and hands-on approach
• Proficiency with MS Office Suite
• Ability to work effectively in a remote environment
• Dedicated workspace to protect PHI/HIPAA information
