Virtual Physician

Responsibilities

• Conduct initiating telehealth-based E/M or Annual Wellness Visits (AWVs) to establish medical necessity
• Identify high-risk conditions and unmet social needs
• Document findings in the platform using accurate coding, SDOH Z-codes, and diagnoses
• Develop and approve individualized care plans aligned with medical treatment plans
• Collaborate with care advocates and nurse care managers to finalize patient support plans

Requirements

• M.D. or D.O. with an active, unrestricted U.S. medical license
• 2+ years of experience in primary care, telehealth, or care management
• Familiarity with Medicare programs, care navigation, and SDOH documentation
• Proficiency with EMRs, care management platforms, and telehealth workflows

Preferred Qualifications

• Broad multistate licensure (IMLC participation or 50-state coverage)
• Board certification in Internal Medicine, Family Medicine, or Geriatrics
