Founded in 2019, Form Health is a venture-backed, mission-driven company partnering with leading employers and health plans to expand access to high-quality, guideline-based obesity and cardiometabolic care nationwide.

Responsibilities

• Lead and scale clinical operations across physicians, APPs, dietitians, and clinical support teams
• Own clinical operating performance including SLAs, capacity, utilization, and multi-state workforce planning
• Set clinical operations strategy and drive practice transformation in collaboration with executive leadership
• Provide strategic guidance to Product, Engineering, and Operations on clinical tools and features
• Lead, coach, and develop clinical leaders and managers while promoting clinician wellness
• Represent Form Health’s clinical organization externally at conferences and client meetings
• Maintain a very small longitudinal patient panel

Requirements

• MD/DO, board certified in Internal Medicine, Family Medicine, Endocrinology, or relevant subspecialty
• Licensure in good standing
• ABOM certification or eligibility to become ABOM-certified
• 5+ years of clinical practice experience
• 3+ years of clinical leadership experience in a scaled, multi-state telehealth or large virtual/hybrid practice
• Proven ability to lead through influence in a cross-functional environment
• Exceptional people leadership skills and commitment to team culture and DEI
